A new digital Occupational Health referral form is now live. Our new referral form looks to streamline the referral process for those accessing Occupational Health and Wellbeing services by providing a single point of access for both self-referrals and management referrals.

By using the new referral form, colleagues can request an Occupational Health assessment or a workplace assessment, share their reason(s) for referral and provide further context. It is hoped that this will aid triage and ensure colleagues are reaching the appropriate support services. Introducing the form will also help improve the quality of referrals through the inclusion of mandatory fields.

The link to access our new referral form can easily be accessed at any time via the Occupational Health and Wellbeing area on StaffZone (please note you may need to be on a Trust device to access this page). Further information on the different kinds of referral available to colleagues and managers can also be found on this page.

Following the introduction of the new form, old referral forms (which can be easily identified as Microsoft Word documents) will now no longer be accepted by Occupational Health. Those who submit an old referral form will instead be asked to resubmit their referral via the new form.
